Michigan's Loan Modification Program

This the list of the documents/paperwork needed if refinancing:

1. Copy of driver's license (both sides) or approved photo ID.  Make sure picture is light enough to view when faxing or copying.
2. Copy of Social Security card both front and back
3. Two (2 - biweekly) or four (4 - if weekly) of your most recent pay check stubs.  (If direct deposit, then bank statements indicating the same.)
4. Social Security award letters, alimony, child support monthly payment(s) if applicable to show income 
5. W-2 for the last two years
6. IRS tax returns for the last two years (all pages and profit & loss statement(s) for the current year.
7. Last two months bank statements for checking and saving accounts.  (Include all pages even if blank.
8. Most recent statement of 401K, 403B, IRA or other retirement plans (all pages including blank pages).
9. Last mortgage statement received from mortgage company.
10. Declaration page of homeowner’s insurance and agent information.
11. Deed
12. Title Insurance (when applicable)
13. Survey (if applicable)
14. Tax spring and summer Statements
15. Payoff of Mortgage or Land Contract
16. Check payable to appraisal company (when applicable).
17. Other:  (Bankruptcy Discharge Papers; Divorce Papers; Explanation(s) of previous foreclosure or financial hardships (if applicable).
18. A year of rent receipts, cancelled checks, and/or statement from land contract holder showing/stating that rent was paid on time (if applicable to show seasoning if applicable). 
19. 3 lines of credit established (billing, credit card, or loan statements as proof - when applicable.
